A INFLUÊNCIA DO FATOR HUMANO NOS CENÁRIOS ACIDENTAIS DE UMA REFINARIA DE PETRÓLEO
Abstract
This work considers a research project concerning the influence of human factor on accidental scenarios that have occurred in an oil refinery, from analyses and evaluation of the causes identified in reports of risk studies elaborated in oil refinery. From revision of literature scientific, that allowed the theoretical recital of the subject that was developed, had been reviewed the basic concepts tied with the area of management of risks and with the human factor and its causes. A methodology was proposal that will be developed from deriving data of the reports of studies of risk of the industrial chemical processes executed during the period of time from November of 2005 to November of 2007 at an oil refinery. Accidental scenarios were analyzed and occurrence causes identified, as like classified in relation to human failure, equipment failure, mechanic integrity degradation (aging, erosion and corrosion), installation/construction, engineering/design, load/crane movement, material incompatibility, abnormal condition of operation and unidentified causes. It has been concluded that human failure was responsible for 23% of the accidental scenarios presented. This result has shown compatible with the report of the most of the authors who were researched and describe through the text.
